New Patients

Medicare and Non Medicare patients, your paperwork packet can be found at the clickable files below. This can be filled out online and printed or you can print a blank copy and fill it out manually. Either way, please fill it out and bring it with you to your first appointment.

What should I bring and/or wear?

Please arrive 15 minutes early (30 minutes if you are a Medicare patient) to fill out paperwork. You may also do the paperwork ahead of time by downloading the forms from our website. If you do this, we still ask that you come in 5 minutes early so that we may get copies of your ID, insurance card, doctor’s referral, and collect any co-pays.

Please bring to your first appointment your ID, insurance card/information, and doctor’s referral.

Please wear loose/comfortable clothing (like what you’d wear to the gym). We also have shorts that you may borrow if needed.

How long are the appointments?

All appointments are 45 minutes long.

Location?

Our address is 14050 Juanita Drive NE, Suite B in Kirkland, WA (on Finn Hill). We are in the Inglewood Professional Center next door to Bartlett Eye Clinic. We are close to St. Edwards Park/Bastyr University and across the street from the QFC/Bartell Drugs shopping center.

Please note that our address is in Kirkland but used to be Bothell before we got annexed by the city in 2011. Our physical location hasn’t changed, however please ensure to put Kirkland in your GPS in order to get the correct directions.
